Complex Care

KD Healthcare provides homecare services to individuals with complex needs.

We believe each and every individual is different and their circumstances and experiences are unique to them. We adapt our care services accordingly in order to fit our client’s requirements and circumstances. Our staff have years of experience working with a wide range of complex care needs including tracheostomy, ventilator, gastrostomy, epilepsy, motor neuron disease etc.

 

Home Care

KD Healthcare is there for you to meet your home care requirements.

We provide personalised care services that fulfil your specific needs.

We ensure your needs are meticulously assessed and a suitable care plan is developed for you. We encourage your chosen loved ones, (if you so wish), to be actively involved in the assessment and care planning process too.

Once your needs are identified and a suitable care plan agreed we will match you with the appropriate Carer who will help you to meet your specific care needs.

 

Using our home care service

After we have received your enquiry, we will contact you to discuss your requirements in more depth and assess how we can support you.

If appropriate, and you wish to proceed, a member of KD Healthcare team will visit your home to undertake pre-service assessment and risk assessments. We will design and cost your care package following this visit and present this to you.

If you accept this care package, we will finalise your care plan with you, in conjunction with your appropriate healthcare professionals and / or your family if required.

We will identify suitable Carers to support you and arrange for them to visit you. We will match the Carer with your interests and personality to ensure you are compatible. If you are happy with your Carer, we will deliver any additional specialised training to the staff in order to provide you with the care and support you need

Once your care commences, the Registered Manager will visit you on a regular basis to make sure we are still providing you with an excellent service and that we are meeting your needs.
